Adding a Patient

This is a step-by-step guide for adding your first patient. Please read the steps below and watch the accompanying demo video. In this demo, the care coordinator, Carrie Corden, adds a patient, Patrick “Pat” Goodheart.

Carrie Corden is a care coordinator for a facility with patients with a caregiver account. Her job is to sign up Patrick “Pat” Goodheart. Pat’s account will be managed by Tori Powers, his daughter and Power of Attorney. 

Add a patient flow

Ways to arrive

There are many ways to add a patient on the PocketRN platform:

  • On the My Stuff page, select Patients and Add a Patient
  • On the Home page, select Schedule an Appointment and Add a Patient.
  • On the Meetings page, select Schedule a meeting and Add a Patient.

Step-by-step

For this scenario, we’ll be considering the flow from Carrie’s, a care coordinator’s, perspective.  You’ll want to consider your situation and adjust accordingly.  The app will guide you through the process.

  1. Follow the steps to add the contact information and the personal information of the patient. We will send them an invitation to confirm this information and to optionally create an account on PocketRN. This invitation will also explain the services PocketRN offers, but we recommend you introduce PocketRN to them ahead of time as well.
  2. Optionally add more caregivers to the patient. 
  3. Designate or add any Power of Attorneys if the patient has them and choose the individual who should sign consents for the patient. If the patient doesn’t have a Power of Attorney, the patient should sign the consents.  
  4. Follow the steps to pass the device to the patient or Power of Attorney if they are present, or to send the consents as a link in an email or text message to be signed later.

We highly recommend scheduling the first meeting as part of this flow if possible. Visits with our nurses are more successful when the patient onboarding timeline is as short as possible. Try to find as early of a time as conveniently possible for the patient. If needed, you can schedule an appointment for this patient later from your app home screen, or you can send a scheduling link out to the patient or a loved one to find an appropriate time.
