How the Nurse For Life Model Is Changing Dementia Care and Elevating Home Care Agencies

Medicare is finally recognizing what home care has known all along. A dementia diagnosis leaves families feeling lonely, confused, and scared. They need guidance, trust, and support exactly when it matters most.

Medicare is funding a nationwide model that brings together home-based care, clinical dementia expertise, and real, day-to-day support for family caregivers. It’s called GUIDE—Guiding an Improved Dementia Experience—and it marks a turning point for agencies across the country.

A New National Standard for Dementia Support 

GUIDE is an eight-year national initiative backed by the Centers for Medicare & Medicaid Services (CMS), designed to shift the dementia care journey from fragmented and reactive to connected and proactive.

Through a partnership with PocketRN and Nevvon, home care agencies can now participate in GUIDE—at no cost to families or to your business. That includes:

  • 24/7 access to your care team and a dementia-specialized Nurse For Life who checks in regularly to monitor health and support caregivers
  • Caregiver training in 11 languages
  • $2,625 in annual respite care services for qualifying families—equivalent to 76 hours of in-home care.
  • Medicare-covered services that help families stay connected to their care team, including physicians, specialists, and trusted community resources

This is the first time Medicare has reimbursed for what home care delivers best: consistency, trust, and connection.

How It Works: A Seamless Partnership, Not More Overhead 

GUIDE is designed to be implemented through partnerships—not paperwork. For home care agencies, that means:

  • No Medicare certification required
  • No new hiring or billing
  • Minimal administrative lift
  • No need to have clinicians on staff

One Nurse. One Family. Ongoing Support

At the heart of this model is PocketRN’s Nurse For Life approach: a dedicated, dementia-specialized Nurse For Life.

What Families Receive Through GUIDE 

When a family enrolls in PocketRN’s GUIDE program, they receive:

  • A dementia assessment, which may include diagnosis

Who is Eligible? 

Any client that:

  • Has dementia (PocketRN can help obtain a diagnosis if appropriate)
  • Has Traditional Medicare Parts A & B as primary insurance (not Medicare Advantage)
  • Is NOT currently enrolled in hospice
  • Lives at home (temporary rehab stays and independent living are allowed)

Note: VA patients, dually eligible Medicaid patients, and palliative care patients may be eligible.
