PocketRN offers dementia support through GUIDE, a Medicare model for eligible individuals and the family caregivers beside them.
GUIDE stands for Guiding an Improved Dementia Experience.

Program support
*Total respite available to qualifying families for 7/1/2026–6/30/2027. Annual respite totals may vary from year to year based on Medicare policy and reimbursement rates.
Eligibility and enrollment
GUIDE is designed for people living with dementia at home.
VA patients, Dually Eligible Medicaid patients & Palliative Care patients can be eligible.
Eligible individuals receive GUIDE support at no cost.
